Thank you for your request.

Please allow two full working days for prescriptions to be processed by the surgery and remember to take weekends and bank holidays into account.

If there are any issues with your request we will contact you.

Important!

This form will only be accepted if you

  • are requesting medication for someone under 16
  • are a housebound patient
  • are ordering acute medication
  • have been instructed by the Practice to use this form

    If you do not fit into any of these categories, your request will be returned and not processed.

    Please request your medication via the NHS app or using your paper request slip.

    Collecting your Prescription

    All prescriptions are sent electronically to your nominated pharmacy and will need to be collected from there. 

    Some local pharmacists will deliver your prescriptions for you, but you have to arrange this with them yourself.

    We can take no responsibility for any arrangement that you may make with another person to deliver/collect your prescriptions.

    When will my Prescription be ready?

    Please allow two full working days for repeat prescriptions to leave the surgery.

    Your nominated pharmacy will need time to make up your prescription after we have issued it. Please allow time for this before you go to collect your medication.
