The practice is now operating a closed door policy, Patients will not be given entry without prior arrangement. On arrival Patients will be asked questions regarding Coronavirus before entering the premises.  ALL requests for appointments will be triaged. Help us to help you by staying away and only contacting the Surgery by telephone.

The building works at Roman Gate Surgery are now complete however access to the site remains limited due to ongoing IT issues that prevent the surgery from functioning fully.  Thankyou for your patience and understanding it is appreciated by the partners and management team of Hicks Group Practice.

New Patient Registration

areamapIf you live in the practice designated (see opposite) area you can join the list.

To register please call at reception and bring your NHS Registration Card if you have one (along with all those of other members of your household who wish to register). You will be asked to complete a form for each member of the household who wishes to register. If you do not have a card then the receptionist will provide you with the appropriate forms.

You will be handed a patient questionnaire for each member of the household to complete and you will be invited to make an appointment for a new patient health check. This will be booked for you to see the healthcare assistant.

At this stage you should also inform us if you are on regular medication, which requires a repeat prescription. If so, you should make an appointment to see the doctor or nurse practitioner so that medication details can be taken and entered onto your records.

You are registered with the practice rather than an individual GP. and may see any of the doctors. You are however, able to express a preference of practitioner and whilst the practice will try to comply, it might not always be possible, in which case we will explain why. You are encouraged to always see the same doctor as much as possible.

If you move outside the practice designated area you will be asked to move to a different practice.

Please keep us up to date if you change your name, address or phone number as we may need to contact you in an emergency.
